Please read this packet to learn how and when (always) to consider assistive technology in the IEP. AT Consideration Form (AT1) is completed at ALL IEP meetings if AT is checked “not needed” on page 4 of the IEP.
AT Request Form (AT2) is completed by case manager or any team member, and forwarded to SPED AT. Action Plan Form (AT3) is an on-going documentation of the student’s plan, progress and team’s responsibilities.
